Telehealth services are provided only where legally permitted and only if you are physically located in a state where the clinician is licensed to practice. Teletherapy is delivered using secure, HIPAA-compliant platforms.
By using telehealth services, you acknowledge and agree that:
- Telehealth services involve technological limitations and potential privacy risks.
- You are responsible for maintaining a private and secure environment.
- Telehealth services are not appropriate for emergency situations.
Our clinicians are licensed to practice only in the State of Florida. By participating in telehealth services, you certify that you are physically located within Florida at the time of each session.
Telehealth services require acceptance of a separate Telehealth Informed Consent prior to the start of clinical care. You agree to notify us immediately if your physical location changes.
6. Insurance, Billing, and Financial Responsibility
Evolve Therapy & Counseling accepts certain insurance plans as described on the Site. Verification of benefits does not guarantee coverage or payment. All coverage determinations are made solely by your insurance provider.
You are financially responsible for all fees not covered by insurance, including deductibles, copayments, denied claims, late cancellations, and no-show fees.
Insurance claim submission is provided as a courtesy and does not relieve you of responsibility for payment. Failure to resolve outstanding balances may result in suspension or termination of services.
7. No Emergency or Crisis Services
Evolve Therapy & Counseling does not provide emergency or crisis mental health services.

8. Intellectual Property
